Salary: Grade 7 £35,745 - £37,336 per annum
Grade 8 £38,223 - £44,428 per annum

The Preparing for Adulthood, Learning Disability and Autism Team support adults, families and informal carers to live as an enjoyable and independent life as possible by working in a strength-based way to listen to and empower individuals, identify their outcomes and explore ways of using their own strengths.
This is an exciting opportunity for a successful candidate to work within a new and expanding team. You will work with a passionate and enthusiastic team to support service users, families, carers and people with lived experience in delivering services that are outcome focused and incorporates best practice.
As part of the wider Adult Social Care service you will also work closely with the five integrated neighbourhood teams, front door team, and Adult Mental Health Services supporting and learning from each other.
This is an exciting time in Rochdale as the service implements service redesign through our transformation agenda and Corporate Plan.

You will have:
- Social Work Qualification
- Previous experience of working with individuals with learning disabilities and/or Autism
- Care Management experience in the Adult Care sector
- Someone who is positive, proactive and creative to make a difference in the lives of people with learning disabilities and autism.
