Rochdale Adult Care is pleased to offer an opportunity for Assessment and Support Planners to join our vibrant and creative authority. We have several generic posts available working across our Integrated Neighbourhood Teams.
The post holder will be expected to have experience of assessment and support planning including offering advice and guidance to service users and carers, within the objectives of the Care Act 2014.
We expect our Assessment and Support Planners to use strength based approaches to promote the independence and well-being of service users and carers, utilising natural support and the voluntary sector where necessary.
The ability to work within timescales and deadlines is necessary as is the ability to write care records and reports clearly and concisely.
In return you will have the opportunity to further develop and progress your skills through undertaking a variety of appropriate training and development courses.
Casual User Car Allowance is payable
Appointment to this post is subject to an enhanced DBS including a barred list check against the adult workforce
